What is mpox?


Mpox is a highly infectious disease that has similar symptoms to smallpox, but it's less severe. Several outbreaks of different clades of mpox have occurred in different countries, with different modes of transmission and different levels of risk. Due to the escalating outbreaks in several African countries, monkeypox has become a Public Health Emergency of International Concern (PHEIC) once again, and warned that a new wave of the epidemic will eventually cross borders and spread globally, according to WHO. And the data collected by them shows that the number of reported monkeypox cases this year has exceeded 15,000, surpassing the total number of cases last year, with over 500 deaths.



How to diagnose mpox?


Clinical identification of monkeypox is the first step in diagnosis, and the confirmation of monkeypox requires a series of laboratory tests. The test method includes Polymerase Chain Reaction (PCR) analysis,Virus isolation through cell culture, Enzyme-Linked Immunosorbent Assay (ELISA), Antigen detection tests.

It is important to note that due to the serological cross-reactivity of orthopoxviruses, antigen and antibody detection methods cannot provide specific confirmation for monkeypox. Therefore, serological and antigen detection methods are not recommended for use in resource-limited diagnostics or case investigations. PCR and virus isolation through cell culture can demonstrate the presence of the virus. Given their accuracy and sensitivity, PCR is preferred for diagnosing monkeypox. Sample sources for monkeypox testing include vesicles, pustular fluid, scabs, and blood specimens. The best diagnostic samples come from the top of skin lesions such as vesicles and pustules or the liquid within, as well as dried scabs.

What are the signs and symptoms of mpox?

Since the eradication of smallpox in 1980 and the cessation of smallpox vaccination, monkeypox has become the most serious orthopoxvirus in public health. Monkeypox is a zoonotic orthopoxvirus with an incubation period typically ranging from 6 to 13 days; the initial symptoms include fever, headache, muscle aches, lymphadenopathy (enlarged glands), and severe fatigue; a few days after the onset of fever, typical generalized pustular rashes appear, which can lead to secondary infections. The case fatality rate of monkeypox is 0% to 11% in the general population, and higher in children.

How does mpox spread?

Person-to-person transmission of mpox can occur through direct contact with infectious skin or other lesions such as in the mouth or on genitals; this includes contact which is · face-to-face (talking or breathing) · skin-to-skin (touching or vaginal/anal sex) · mouth-to-mouth (kissing) · mouth-to-skin contact (oral sex or kissing the skin) · respiratory droplets or short-range aerosols from prolonged close contact The virus then enters the body through broken skin, mucosal surfaces (eg:oral, pharyngeal, ocular, genital, anorectal), or via the respiratory tract. Mpox can spread to other members of the household and to sex partners. People with multiple sexual partners are at higher risk. Animal to human transmission of mpox occurs from infected animals to humans from bites or scratches, or during activities such as hunting, skinning, trapping, cooking, playing with carcasses, or eating animals. The extent of viral circulation in animal populations is not entirely known and further studies are underway.

Most people with mpox will recover within 2-4 weeks. Things to do to help the symptoms and prevent infecting others: · Do stay home and in your own room if possible · wash hands often with soap and water or hand sanitizer, especially before or after touching sores · wear a mask and cover lesions when around other people until your rash heals · keep skin dry and uncovered (unless in a room with someone else) · avoid touching items in shared spaces and disinfect shared spaces frequently · use saltwater rinses for sores in the mouth · take sitz baths or warm baths with baking soda or Epsom salts for body sores · take over-the-counter medications for pain like paracetamol (acetaminophen) or ibuprofen.
